Immersion Outpatient

Immersion Outpatient provides structured, evidence-based outpatient treatment for individuals recovering from substance use disorders, mental health conditions, or co-occurring diagnoses. Located in Delray Beach, Florida, our outpatient program supports patients as they transition from residential care — or, in some cases, begin treatment without requiring inpatient admission.

Immersion Outpatient is part of Guardian Recovery’s Immersion continuum of care, designed to meet patients where they are and support long-term recovery.

What Is Immersion Outpatient?

Immersion Outpatient offers clinically guided treatment without overnight stays, allowing patients to live at home or in supportive housing while continuing therapy and recovery work.

This level of care is ideal for individuals who:

  • Have completed residential treatment and are stepping down

  • Need structured support while returning to daily responsibilities

  • Have mild to moderate substance use or mental health needs

  • Are clinically stable and do not require 24/7 supervision

Treatment plans are individualized and adjusted as patients progress.

The Immersion Continuum of Care

Residential to Outpatient Treatment

Many patients begin their recovery journey at Immersion Residential in Boynton Beach, where they receive intensive, 24/7 care. As stability improves, patients transition into Immersion Outpatient in Delray Beach to continue therapy while gaining greater independence.

Typical Treatment Flow:

  1. Immersion Residential (inpatient stabilization and intensive therapy)

  2. Step-down to Immersion Outpatient

  3. Continued recovery through PHP, IOP, and outpatient services

  4. Aftercare and long-term support

This seamless transition helps patients maintain momentum and reduce relapse risk.

Can Someone Start Directly in Outpatient Treatment?

Yes — in certain cases, individuals may begin treatment directly at Immersion Outpatient.

Outpatient treatment may be appropriate for those who:

  • Do not require medical detox

  • Are not experiencing severe withdrawal or relapse risk

  • Have stable mental health symptoms

  • Can safely manage daily responsibilities while attending treatment

  • Have a supportive home or living environment

Our admissions team conducts a clinical assessment to determine the most appropriate level of care, ensuring patient safety and the best chance for success.

How long is the treatment program?

Treatment length is individualized based on clinical needs, diagnosis, and progress. Some individuals may begin with medical detox (when appropriate) followed by residential treatment or outpatient care, while others may start directly in an outpatient setting.

Treatment plans evolve over time and may include multiple levels of care as part of a structured recovery process.

Immersion Residential provides 24/7, structured inpatient care for individuals who need a higher level of support, stabilization, or medical oversight.

Immersion Outpatient offers PHP, IOP, and outpatient treatment for individuals who are clinically stable and able to live at home or in supportive housing while attending treatment.

Many individuals transition from residential treatment into outpatient care as part of their recovery journey, though some may begin treatment directly in outpatient services depending on clinical need.

Yes. Guardian Recovery’s Immersion Programs provide integrated addiction and mental health treatment, including care for individuals with co-occurring (dual diagnosis) conditions.

Rather than treating addiction and mental health separately, our clinical teams address both simultaneously when they occur together.

Treatment plans may include a combination of evidence-based therapies, such as:

Therapies are selected based on individual needs and adjusted as treatment progresses.

Yes. Medication-Assisted Treatment (MAT) may be offered when clinically appropriate to support recovery from substance use disorders or manage mental health symptoms.

MAT is always provided as part of a comprehensive treatment plan that includes therapy and clinical oversight.

Family involvement may be incorporated when appropriate and clinically beneficial. Family therapy and education can help improve communication, understanding, and long-term recovery outcomes.

A continuum of care means individuals receive treatment across multiple levels of support as their needs change — without losing continuity with their care team.

This may include moving from residential treatment to outpatient services, or adjusting treatment intensity over time to support long-term recovery and stability.
